Auto CPAP Optimization Guide? ResMed AirSense 10 AutoSet

I currently have a F&P machine on trial, but have the ResMed on order. I will have to do the optimization of it, as it is an on line supply only company. I'm quite OK with that, and have the full Clinical manual for the machine. So, I know how to change the settings, but what I am not sure about is how to optimize them. The manual provides very minimal guidance. So what I am looking for is more an optimization guide. For example one that would answer these kind of questions.

1. What do you set the minimum pressure at and why? What are the consequences of having it set too low, or too high?
2. Same questions with the maximum pressure? Is there any benefit to setting it lower than the default max of 20?
3. EPR benefits? What if it is set too high, or too low? This one I suspect is quite controversial...

Is there a link you can provide to a guide with this kind of info? So far I have seen lots of this is how you change the setting, but not so much on why. I have Sleepyhead downloaded and have used it with my wife's ResMed S9 Auto, so I am familiar with the data it can provide. I can't use it with my trial F&P machine as it is not supported yet, but I will use it with the AirSense 10 when I get it. 

Perhaps that is my real question. How do you use Sleepyhead data to optimize your CPAP for comfort and best performance?

Does the 80/20 insurance really save over a self purchase?

Was just wondering if going thru a local DME really saves anyone any $$$ as compared to an internet purchase. Your deductible and 80/20 deal with insurance and possible higher of a machine locally.

Hello all,

As my mom is leaving the country I would like to prepare necessary replacement parts for her for the resmed airsense 10. I see the guideline about the replacement for mask cushion, water tank, headgear, air tubing etc. Per your experience, how often do you need to make the changes? I see if follow the guideline it is quite costly to change them regularly.

I bought the ResMed machine in US for my mom use in China. Currently i use myair iphone app to monitor her sleep data while she is in US. Does anyone have experience that the sleep data can still be available in the app I have here during  herusage outside of US or there is no other way? thank you so much

Newbie - seeking roadmap for Dx and Tx

Hi Folks,

Newbie here.
  • Male, age 42.
  • Physically fit, low body fat.
  • Blood pressure on the low side.
  • Have deviated septum, but not severe.
  • Have IBS, symptoms of which interfere with sleep (mainly pain/discomfort keeps me from falling asleep).
  • Wake at least once per night to pee, then remain awake for 30-60 minutes. Sometimes wake multiple times.
  • When I wake after 6, 7, or 8 hrs in bed, I feel the heavy weight of fatigue and just want to sleep more, but have trouble returning to sleep.
  • Regular daytime fatigue, can easily lie down for a nap and fall asleep during day or eve. Scored a 6 on the Epworth scale, which I understand is not very high.
  • I use blackout curtains, eye mask, earplugs. Home and neighborhood are very quiet.
Due to [insert rant about sh*tty state of U.S. healthcare system] a proper sleep study in a lab is cost prohibitive. This may change in the future.

So for now, what kind of roadmap can I follow to at least narrow down my diagnosis?
  • Check blood O2 levels?
  • Try CPAP machine (as noted on forums, much cheaper than a sleep study)
  • Other things I can try?
My goal is to find short term clues and relief, while acknowledging I need a full sleep study in the future.

Hello All, 

I was finally able to download Sleepyhead... whew... I have no idea what I'm doing, as this is an old Mac Air, I'm not proficient in using Macs... so please bare with me.

*I know I need to organize the charts... I'm just hoping I can find them, at this point to upload (it's taken me all day to do this).  I wanted to get some feed back... I will organize later.

Pitiful Experience at a DME

I moved, and have been on the hunt for a good DME.  I called one today and asked if they had CPAP masks.  The lady had to put me on hold to find out. (This was enough to tell me to cross them off my list, but I don't see too many DME options near me, so I figured I may as well give them a chance.) They do.  So I went in to see what they've got.

I walked in and asked to see their CPAP masks.  The lady handed me two pieces of fabric.  I realized they were CPR masks.   Too-funny

I pointed out her error, and asked again to see CPAP masks.  After quite a while in the back (I almost walked out, it took so long), she came out with 2 tiny tupperware tubs.  Due to my extensive(?) research and experience, I identified that there were 3 different ones: 2 FFMs, and 1 nasal mask.  They had no branding, and looked ultra-generic.  There were no sizes, no options.

If you could get a CPAP mask out of a giant gumball machine, these are the masks you'd get.

It was so sad.  Mainly, I felt very sad for anyone who needs CPAP supplies and this is all they're offered.  This has got to be just one of many reasons so many people give up on CPAP and just suffer with untreated OSA.

A10 side cover vs humidifier - which for travel?

I just looked at some images of the side cover for the A10s. It looks almost as large as the humidifier tub!

Does anyone use the side cover vs the tub? If so, is it as large?

I'm looking to cut down on bulk in the bag so I can use the space for misc. I've not traveled with this machine yet.

Hello all,
I am posting two images from SH below.
This is the second night in a week that I have had about 15 mins flagged as Cheyne Stokes Respiration. The AHI is also quite high for me. I have been averaging <3 typically. The first time I saw this CSR flag it lasted for 19 mins and my AHI for the night was 5.8.
I'm not sure what caused the large leak before this event but that is not typical. The previous day that I had the CSR flag there was no such leak.
The breathing pattern seems abnormal to what my typical apnea events look like because I usually don't have so many in such a condensed cluster.
Could this actually be CSR? If so, what exactly does that mean because googling it gives lots of scary information.
Any feedback is appreciated.

What is the different between a Resmed Vauto and a 10S Bilevel machines?  I saw a good deal for a barely used one that I'm considering getting to see if it will help the waking up at night (suggested by another forum member).  But I can't figure out what the difference is.

Hi.
During my sleep study 2 months ago I was found to have an AHI of 32.
I'm using a DreamStation Auto CPAP w/AFlex plus humidifier set at 5-12cmH2O and my AHI has dropped to between the mid 2's and low 5's.
I'd like to actively manage my apnea to bring these numbers down even further so, after registering and reading several posts, I downloaded and began using the SleepyHead software. 

The export to CSV function is great however I'm interested in seeing a little more detail rather than just the daily summaries.

Is there any way to export data from SleepyHead by the time of day that each event occurs?
I would like to use Excel to chart the data over several weeks or months to determine what events happen at what times in order to observe any trends based on time of night. (ex. do more hypopneas happen between 12am-1am or 4am-5am).
With this information I could more closely monitor and modify sleep variables based on time.

I'm thinking of getting a backup machine just in case and I'm just wondering what people's experience has been.

Oximetry Pulse Relevance?

How relevant is the pulse portion of oximetry data? 

I have two overnight oximetry reports I made. The oxygen portion is a svelte 1 or 2 events per hour but the pulse rate portion has 20+ events per hour. 

The reason I think the pulse is relevant is that my polysomnography reports show around 15 to 25 spontaneous arousals per hour and that number matches up too well with the pulse rate events. I think the pulse fluctuations may be related to my excessive daytime sleepiness but none of the doctors I've seen know anything about it.

Some background about me, I have mild sleep apnea, around 7-9 AHI without CPAP. I was diagnosed with Narcolepsy type 1 on November 2017. I finally got my CPAP just two weeks ago and I'm as tired as ever.  I've been averaging about 4 AHI since I got the CPAP so I'm aware I have some more fiddling to do with it. The oximetry reports I did were also before I had the CPAP.

AHI Accuracy from Brand to Brand

My wife has been using a ResMed S9 Auto for about 3 years now. She was diagnosed with an AHI of 67 or so. Probably about 50% of her AHI numbers are under 1.0 and virtually never over 2.0. I was just diagnosed with an AHI of 37, and am currently on a trial F&P machine. It is a very recent model and seems to have all the bells and whistles. However after 4 nights my AHI has ranged from a low of 2.7 to a high of 6.0. The machine is not supported by SleepyHead yet so I can't get into the detail of where my issues are.

This is a bit embarrassing as my starting AHI is nearly half what hers was, and I can't seem to get numbers anywhere close to what she gets. The machine setup is essentially the same for pressures and both are in Auto mode. So I am wondering if the AHI number is actually accurate/reliable enough to compare between machines of different brands? Or, is it just a relative number within the brand and useful only for determining relative treatment effectiveness?

I have a question that seems like it should be one of the easiest things in the sleep apnea world to find out, but after 3 hours of googling up and down the internet, apparently not so.  I finally found this forum and you may be my only hope.

I have a Devilbiss Intellipap auto-everything smart flex cpap machine.  I have recently retired and have been sleeping later in the mornings.  Whereas during my working life I got up between 6-6:30 and almost never got over 6 hours sleep a night, I am now trying to get 8 hours and am getting up after 7:00 most mornings, sometimes after 8:00 thank you.  Did I mention I was retired?

Anyway what I run across most mornings now is my machine says I slept 1-2 hours and all my readings are for that time period, so almost useless.  The machine apparently resets to the next day at 7:00 am.  I want to change that.  I have the clinical manual and can change almost anything on the machine but that.  Why would they set that so early and why would they not specify how to change it?  Even working people work all kinds of shifts, not everyone wakes up before 7:00 every day. This machine also does not let you go back to the prior day like my previous machine did. That would almost solve the problem, but the Intellipap only gives you 1 day, 7 day, and 30 day readings.

So, can anyone help me with that?

I'm getting a used ResMed AirSense 10 AutoSet and here's what I plan to do once I get it:

1. Sanitize/clean the machine, hose, and water chamber.
2. Erase all the data on the machine.
3. Set up the pressure and other settings to my prescription setting/preferences and make sure the time and date are both correct.

Is there anything else that I'm forgetting? Please advise. Smile Thanks!
