Overslept last night, forgot what day it was *sigh*

Last night, I slept nearly 10 hours. When I woke, for some reason I thought it was Saturday not Friday, and was late for work.

Here's what it looked like.

When is it time to get another sleep study done.

When I took my first sleep study, I had a horrible sinus infection which blocked almost all airflow through my nose. I ended up with a AHI of 14, with 42 during the REM sleep from that study. Since then I have had sinus surgery and have had drastic improvements in airflow through the nose. I have been slowly dialing down the max pressure of my APAP and have noticed no change in AHI (<1.5 nightly). I am now at a min of 5.4 with a max of 9.0. Avg is 7.0.

Should I go get another study to see if I still have apnea after the surgery? I will have to pay a little out of pocket, but worth it if i can ditch the machine. Should I continue to dial down the pressure if I still have good results?

Thank you for your time.

Why did i wait, really, woke up awesome!!

First off, I have no idea why i waited so long to do therapy. I had been diagnosed years ago and never ever even consider Therapy. All i can say having fixing things here and their and trying to get this right. I have been feeling awesome. more productive, awake, alert.. Feels like i can focus more and thats just with 3 days of therapy.

If your feeling anything that is negative about trying to get this right, push through. The world is amazing when you wake up.

Here is last nights info, Day 3 and i actually slept the whole night and didnt wake up once. That is a first in almost 8 years. 6 am bright and shine and i feel great. I did have a really bad stuffy nose if that contributed to anything i dont know, but still woke up great.

The first two night issues were mask related, again i was issued a small mask and switch to large and DAMN, what a difference!!

Seem to be moving backward

Since beginning therapy a few weeks ago, I have found that I wake less frequently, but still up a number of times during the night.

After a few days, things were progressing on API, but have since adjusted to try to improve.

Original setting was 5 cm - 10 cm with auto ramp. Eventually adjusted by my therapist from 7.2 to 16 with auto ramp. Am attaching the best result from that time.

Lately in an effort to get better results, I have moved to 8 cm to 16 cm with no auto ramp. It's deteriorating as you will see below. Leak rate is up, but I don't think its mouth leak - no dry mouth.

Any suggestions are most appreciated.

Good Sleepyhead chart posted as well as last night's.

Hi!
I downloaded EncoreBasic ver. 2.4. from this forum and installed it. Everything is fine, but I can only view ALL DATA when the window to choose an option ALL DATA and DATA RANGE is diplayed. When I choose data range option within any date I get the response : "Unable to generate the requested compliance download report".
I started to use the new S/T SYSTEM ONE machine 60 SERIES on 8. jul. 2015, but the data downloaded from my SD card (P-SERIES) it starts from 26. mar. 2015!?
Before I had an older S/T SYSTEM ONE which worked very fine with ENCOREVIEWER 2.0.
Any idea to solve that issue or maybe link to ENCOREVIEWER 2.1.
PLEASE HELP!Thanks

My Old House

I should have known not to start it but I did.

There's the original parts of the house. They are sturdy, strong, well built. Then there's the Idiot who came along later. Anything the Idiot touched is borked. And most of them we stand back and scratch our heads as to why he did it this way. Or why they even bothered.

The house was not built with a bathroom so what was done was they moved the doorway to one of the rooms, added a wall, and closed off the end of the hall they created. The bathroom is itty bitty at 5x5. Most of that work was not done by The Idiot. But at some point, the Idiot had a hand in it. There's a kind of false wall between the one room (my office) and the bathroom. It doesn't need to be as deep as it was made and we don't think it is "original" to when the bathroom was added on. Then paneling was added to the hallway, including the ceiling. The hallway is extremely narrow and, at 5'5", I can touch the ceiling and both walls. So the paneling has always been something we've wanted to get rid of. With the pup coming, and with my chair having banged the crap out of the corner where I come out of the office, I decided I would take the paneling down from that wall so she would not chew on it.

What a mess.

The paneling came off easy enough. It was barely held on. Underneath it was several layers of wallpaper, two we had not seen before (or had not in a while). The first two came off easily as they were not glued in place, but nailed. The third is glued on but the wood underneath it is rough in places so it is coming off in smallish chunks. Underneath that paper is newspaper. We had the same thing in the office. We have the entire from page of the Asheville Citizen Times that declared President Harding Is Dead.

I need to clear off that wallpaper and newspaper from about knee high down (puppy reach). In other places where we have encountered this stuff, warm water and making scratches in it was all it took.

Still, a good project for a cold weekend.

If interested, PM me and I'll give you my Instagram link. You can see the pics there of the newspaper bits and the rest.

Resmed Adapt ASV question

I have been using the Adapt for a while now, it is the only machine that I could tolerate. However I am still not getting good sleep and have been doing some research on Tidal Volume and Respiratory rates.

Some of the reading I have done leads me to wonder if it is possible that the Adapt is forcing me to breathe at a higher rate than would be natural for me.

For instance I often wake up in the night, on the machine, and find that I am breathing rapidly. The next morning I feel light headed like I have been hyperventilating.

Does anyone know if the algorithm of the Adapt has a set breath per minute rate? I have read some users of ASVs write about training the machine to follow their breath, is this what I should be doing?

Any thoughts??

Mask Fit?

Hi. I just received a Large size Amara View. I've been using a Medium ever since I began cpap (6 weeks ago). Haven't used it to sleep yet, but it feels good so far.
Just wondering why neither my sleep doc nor the company ever really "measured" me with that thing used to measure you. Were you all measured?

High AHI (7.5) and don't know why

So I was diagnosed and provided with a Philips Dreamstation two weeks ago. After going through a mask fitting session, I was given a ResMed Airfit F10 full face mask. I will admit it's a little difficult getting used to breathing air warmer than what I would like, but I've made sure to use the machine every single night for the entire night. My first couple of nights my AHI was 9.5, and then dropped to 6, and then one night where it was 2.3! Then I noticed I was having a lot of irritation on my face where the mask makes contact. I wash my face well every night and also wash the mask every morning. I went back to the sleep clinic and they gave me a Philips Amara View which fits quite differently. That mask worked well for a few nights, and then I was unable to get a good seal, and my AHI was about 9.5, so I went back to the Resmed mask and adjusted it a little looser. I still get a good seal, but it doesn't seem to irritate my skin, but I cannot get my AHI below 7.5. My pressure is set from 6-12 cm H20 and I keep the humidity relatively low and 2-3. Any suggestions? I'm going to call my doctor on Monday to ask them what they suggest, but figured I'd check here first.

Please find attached my imageshack pic of last nights results.

The reason I am querying it is that i was in the sleep/wake stage for about 30 minutes at the end of the night yet there is no SWJ.

Is my machine reading things properly? it's brand new and I did up the min pressure by .5 the other day. I've also had quite a few nights well below 1.0

What do you think?

Pressure experiment

Just thought I'd share something I found interesting... A few months ago I experimented with the pressure settings on my CPAP machine. My prescribed setting is 10. I switched my machine to auto mode and set the pressure for 10-13 and was surprised to find that my machine delivered a pressure close to 13 most of the night. But, my AHI never consistently decreased enough for me to feel that having the higher pressure was worth it so I put it back to 10. I was titrated to a pressure of 10 in my sleep study last year.

Fast forward to a few nights ago when I was having trouble sleeping and did not feel like wearing my mask (this is very unusual for me). I decided to turn my machine down from 10 to 9 to see if I could get more comfortable. It turns out I've gotten some of my lowest consistent AHI's on a setting of 9 than I had on 10 or on auto.

As a side-note I do have some problems with CA's when I experience changes in pressure. But, I was surprised to find a pressure of 9 seems to work well for me when the machine went up to 13 in auto mode. Could this be due to the CA's I experience with pressure changes? Sorry, I don't have graphs attached but I thought some people mind find this interesting.

I had my sleep study and titration with nasal mask. Do the results possibly differ if they used a nasal or full face?

Info on INSPIRE

Hi again, My sleep apnea Doctor referred me to an ENT Doctor to see what the cause of my severe apnea is (AHI 57). He thought perhaps I had an enlarged lingual tonsil as he could see nothing else obvious. I had a CT and he said it didn't give him any information he could use to tell me the source of the problem. He thought perhaps it was the base of my tongue.
He suggested I see a nearby specialist in Iowa City - (90 miles from where I live) to see if I am a candidate for Inspire, and he suggested I read up on it Online and ask on the Forum to maybe get some first hand opinions.
He didn't know about insurance coverage and said to find out from their office. Do any of you know if Medicare would pay for it? I know it's government approved, so I would think so.
So if any of you forum members have Inspire, I would love to hear about your experience with it.
One thing I would wonder about is how do you know it is working properly? I suppose they would check it after installation, but then it seems like it would just be blind faith. But I suppose blind faith comes into play with many things associated with our Heath. (We have faith our water is not poisoning us and that the air we breath is safe.) He compared it to trusting your pacemaker.
So thanks in advance to you Inspire people out there. Maybe once you got Inspire you stopped visiting the Forum, but I think there will be some info coming forth.

ASV/ Central apena - Time for a second opinion?

$

I was recently diagnosed with central sleep apnea, then had a titration study for cpap then a second for ASV. The study was originally to look for plmd, not apnea, I usually sleep straight through for the first 6 hours of sleep then start to jerk awake as my Parkinson's meds wear off. I take a nap once a month max. My sleep doctor and I are currently in disagreement, he wanted to give cpap a try for a month to see if the centrals that existed before treatment will disappear in time. I feel that since prior to asv titration he was warning me away from cpap that's a bad idea and I'm starting to get the sense he doesn't have that much experience with centrals and that his technician doesn't either (the practice is so small that there is only one technician covering two locations)

I want to make clear I am open to using a machine as long as it's not objectively making things worse.

Complicating factors-
-I have delayed sleep onset disorder, if I get to sleep before 3am it's a great night, wake up at all my sleep studies was 6am
-I take pain meds and have been diagnosed with Chiari one malformation as well as Parkinson's disease all of which cause apnea. My sleep dr thinks the pain meds are the cause of my apnea, my neurologist thinks my Parkinson's is the cause ( becuase a few years ago I had sleep study and my AHI was 0.5, at the time I was taking twice as much pain meds as I am now and the only thing that's changed is the Parkinson's) I freely admit I don't know who's right.

So with all that said here's the numbers from my studies

Sleep study 2- cpap
Notes- slept 3 hours night before- pretty relaxed- was instructed to take double my usual dose of Ativan- also this time the head of the bed was raised like my bed at home is- breathing against the pressure didn't bother me at all- the sensors were more bothersome than the mask- I kept wondering if it was even blowing because I couldn't feel it- however it did make my ears pop and I swallowed so much air that my appetite was decreased for two days- and I am currently struggling to keep my weight up

TIB-485 min
TST-285 min
AHI- 14.5
Low Sp02- 90%
RDI- 23.3

Arousal index- 28.5
Awaking index- 7

Sleep study 3 asv titration
Note- slept 9 hours night before- had a very upsetting conversation with my mother who has dementia immediately prior to arriving at sleep study- the conversation left me wondering if she was hallucinating again. I also found the oscillating noise of the machine hard to sleep through- the next morning I thought I hadn't slept at all- however I did not struggle with ear popping or swallowing air. I did have average leak rates of 35%
TIB- 427 min
TST- 185

My sleep felt more fragmented at all of my studies than it usually does. I had a headache for days after studies number 1 & 3 indicating to me they were not my normal.

So at my last appointment my Dr suggested that we try cpap and hope the centrals resolve themselves- his argument was I'd slept the soundest during that study- I pointed out I was the tiredest during that study, and can't afford to have my calorie consumption under a thousand a day due to air swallowing. He sent me home with a recording oximeter to try to get data to support insurance covering overnight oxygen by itself. But with my O2 numbers so good that's unlikely.

To me it doesn't seem sensible to go on cpap for one fewer apnea a night and a difference in O2 stats that could be explained by the head of my bed being raised (the de-sats in my first study was during my one obstructive apnea)

So am l wrong about giving cpap a try for a month? If oxygen is approved should I give that a try? Or is it time to seek out a second opinion and find a sleep center with more experience with ASV titration? I'm just so frustrated with the whole process that I want to give up and focus on my other health issues for right now (I was first referred to this dr in JULY) and I know that frustration can lead to poor decision making, so I'm asking the opinion of people with more experience in this area. Am I thinking it's time for a second opinion out of pure frustration or because the numbers actually support a second opinion being a good idea?

Thank you so much for your time and for letting me vent.

Just joined a couple of days ago, so hello to all. I'd better add thanks to that as well, as I have learned so much and had many questions answered already. My wife has also been diagnosed and will be getting a her unit i a couple of weeks.
I do have a question even though I have been using my bipap for just of couple of days. Did anyone else, in those first few days of using your machine, feel like you just ran your first half marathon or have been doing deep breathing exercises for a few hours. Not a pain in anyway, just a knowledge that you have done something your not used to. Just curious. As soon as I get a card reader (the new computer did not come with one) I will look at the sleepyhead data.

Howdy, all.
New cpap user here. I got my machine almost two weeks ago and have been using it every night. I have installed Sleepyhead and have been importing the data regularly. Just curious about which numbers/graphs should I be concerned about. The AHI number is what I see mentioned the most, but it's graph is not super meaningful. Also, if posting an image, which of the graphs should be included?

Anyway, glad I found this board. Lots of good information and the people seem very knowledgeable and helpful. Looking forward to learning more from you all..

ADVICE ON THIS DATA

Any Significance?

So... in my quest to stop mouth breathing I tried an experiment. I used a cervical collar for four days and taping for four days. (chin strap was totally ineffective)

The days I used the collar my AHI has been <.05 but I am mouth breathing a lot. Enough to make my mouth feel like a desert and my throat sore and I am snoring. Leak rate isn't horrible tho.

On the days I taped my AHI has been 1.8-ish. No snoring and no mouth breathing. No dry mouth and my chap-stick is intact from the night before. Leak rate is barely above the intentional rate.

I was surprised to see a lower AHI with all the mouth breathing. Is it really a bad thing?

Which way do I go?

Several years ago, I was diagnosed with mild sleep apnea by sleep studies at my local hospital. The first night went well, but the second night I stayed there was a nightmare and one of the workers was actually fired over their treatment of me. So I got off to a bad start. Also the CPAP mask and equipment supplier was miles from my house and they were not willing to spend enough time with me to find what mask would be best. I am claustrophobic, evidently I have a small face and I am a mouth breather. I used my device for a few months and it did not seem to be helping and it was driving my husband nuts. So I gave up.
I cannot function as tired as I am and I know for general health reasons, it is important that I get back on treatment. After the first of the year I start on new insurance and I am determined to make it work this time. I found a CPAP supplier near my home and actually "browsed" around their store a few times until I was sure how they treated their customers. They recommended a new doctor who would let me do a home sleep study and as soon as insurance settles in, I will make an appointment for that. I love my husband dearly, but he is a very restless sleeper and a horrible snorer (no apnea) and I get no rest with him, plus the CPAP noise bothers him, so the time has come for me to set up my own bedroom. He's not happy, but is starting to understand how important it is for me to get some rest.
I am starting from scratch and have been shopping for a new mattress and really like the memory foam style. I sleep on my back (trying to break that habit) and I have a bad back and neck from a car wreck years ago. The memory foam ones in the store feel wonderful, but my husband insists this is the worst kind for someone with sleep apnea. Not sure what makes him an expert or if he is still trying to find a way to keep me with him all night, so I am asking this forum what experience others have had with different mattress types. Price is a consideration and I hope to take advantage of Black Friday sales. Since I do not have a new doctor yet, I cannot ask him.
Any suggestions from the group? Thanks!

Software for PR System One BiPAP autoSV Advanced

I have had this machine for a week now and could not get downloads from my SD card to load into SleepyHead (v.1.0.0-beta-2) or Encore Basic (v. 2.4..8.0). I assumed it was a defective machine and failing to write to the card, even though the telemetry worked. Today, finally, my local DME tech found the time to listen to the problem and contact Respironics tech support.

She also downloaded all the week's data from my SD card into her Encore Pro program with no problem. Whiskey Tango Foxtrot? Both Sleepyhead and EncoreBasic state clearly that they DO support PR System One BiPAP autoSV Advanced. Long and short of it, the Respironics tech claimed only that neither of those two programs would work with the "new" autoSV Advanced machine, and DreamMapper was my only choice, since I could not get access to EncorePro which was for provider offices, only.

I registered and took a quick look at DreamMapper and want my Sleepyhead back.

(Speaking of semi-licit EncorePro, I have obtained the program file from the source, but when I try to install to the "One Computer" option, it will not allow this and will only go to the network version which frightens me away with the numerous questions that I have no idea of the meaning.)

Has anyone else had this problem with the autoSV Advanced and Sleepyhead/EncoreBasuc?
