Pressure increase after interesting flow rates

I have a dreamstation auto, use heated hose and humidifier, ResMed P10 nasal pillows.

Machine settings are 9.0cm floor, 13cm ceiling, 20min ramp from 7cm, A-Flex is set at 2.

Last night's AHI was the best ever but aerophagia side effects were horrible.

Could someone look at the general zoomed in area and then the more zoomed in area and give me an idea of what was going on at the time the pressure increased so much?

We are somewhat familiar with CPAP. After brain surgery & radiation, my husband is using one & somewhat in love with his unit.
Unfortunately, we are years away from covering our medical debt for his care and adding to the pile of debt is not my A-Plan.
I recently went for much overdue checkups. My PCP ordered blood labs and a sleep study. I was able to set up a payment plan for the sleep study but was not able to do the same for labs so I had to delay those until my sleep study is paid for (6 months). 
The sleep study group wanted me back for an additional in-Lon study but BCBS denied & ordered an APAP at home. BOTH companies require a hefty down payment to get the machine that will then titration my settings (not done at sleep study). I just cannot afford it until I get the sleep study paid off.
I saw a machine on eBay but, without knowing the titration numbers, can I figure out where to set it? Is it dangerous to try this myself? Is it doable? (My husband’s unit is titration remotely) 
I realize this is not a spectacular choice, but if I want something before next winter.
I look forward to hearing from everyone

This week, I received a Phillips Respironics Dream Station.  (It replaced my 9 year old faithful friend, who expired from daily use.)  The DME "coach" for this new CPAP tried to make it sound like the cell modem that was pre-installed should NEVER be removed.  In the most condescending way possible, she tried to list the benefits of the modem...............as SHE sees them.

To clarify, I'm NOT a person who's in denial about the need for CPAP.  I've been a faithful user since the sleep study showed 96 AHI, before I got my first machine.  I fully recognize the life quality improvement that it has made to my life.  Being 6 days without one was miserable and the bureaucratic delay was taxing on my patience.

The Dreammapper software is a benefit to me, personally.  I appreciate being able to read the stats on my computer and via Bluetooth on my smart phone. 

BUT, I resent the fact that a cellular modem can triangulate my location at any moment or wherever I choose to travel.  I also resent the fact that the DME chick can change my settings, at will, and that they will send reminders and supply replenishment messages through my machine.

With my first machine, I was fine with an SD memory card, and the new machine still has one.  But adding the spy modem is just too much.  I'm relieved to know that this one CAN be eliminated instead of the ResMed's AirSense 10 that is embedded.

That thing makes me think of a familiar tune:
       It knows WHERE you've been sleeping,
       It knows if you're awake,
       It knows if you're compliant or not,
       So remove it for security sake!

If I pull it now, will the insurance have an issue, since it's it's a replacement with a 4 month payment?  

When I pull it, will the Blue tooth still work for my smart phone?

Please understand that if you're one of the posters who cannot see the privacy / security issue, I'm not interested in a philosophical debate.  It's going to be yanked out either now, or after it's all paid for in four months.

What I'm looking for are practical pros & cons of what I'll have to deal with WHEN I remove it from the machine.

Good day all!  I'm writing on behalf of a friend who just this week received her home study results.  The catalyst for her obtaining a home study was me talking about my own experience with apnea and her symptoms.  It wasn't much of a surprise that she was diagnosed with severe apnea via the home study.

I have insisted that she educate herself by way of this forum and she is doing so but is also feeling very overwhelmed at the moment.  I obtained her permission to post her sleep report for comments. 


Total Apnea-Hypopnea Index: 43.6
Total Apneas: 85
Total obstructive Apneas: 75
Total Hypopneas: 99
Total Central Apneas: 9

Impression/Diagnosis: Findings are consistent with Severe Obstructive Sleep Apnea

Recommend Auto Pap with pressure ranges 5-20 cmH2O with download or facility based PAP titration

Oxygen desaturation index: 45.9

Patient qualifies for O2 with an SaO2 89% for 5 mins or greater

Heart Rate Min-Max 64-100

Severe Hypoxemia was noted

She is seeing her Dr. on Monday.  The reason I wanted to post her results was the reference to severe Hypoxemia.  She lives at an elevation of 7200 feet.  I'm interested in other members experience living at high elevation and Hypoxemia.  I've already told my friend to make sure the Dr rules out other conditions which could be contributing to the Hypoxemia.  But I'm curious.  Have other members living at high elevation had such high O2 desaturations caused solely by apnea?  Any insight would be greatly appreciated.

I've had my Resmed Autosense 10 for about 10 days. I've posted a few threads on here. Just about everyone that responds tells me to install Sleepyhead. I guess the screenshots will allow people to help me with problems. I'm trying to send my first screenshot

S10 Time says 51:00

When I check the time on my S10 it shows a time of 51:00, no matter what time of day I check. I can set the time, but as soon as I exit the menu, and re-enter, it reverts to 51:00.  

My SleepyHead reports show the correct time. 

What gives?

Some folks have it and some folks don't. It can sometimes be cured by making sure the hose attachment clicks well into place at the top of the machine, or can sometimes be alleviated by using electrical tape around a leaky hose or connection, or muffled with some other creative solution.

This assumes the humidifier attachment is there, even if not turned on, as the machine without it is stand-alone noisy.

Last night, after months of trouble-free, quiet use, my DreamStation started making the oft-described whirring motor noise. Why? What was the variable? Thinking-about

Well, I had overfilled the humidifier tub before putting it into the machine. Some water had spilled out of the internal tube-like structure located at about the halfway mark inside the tub. I had cleaned up the spill, but still put the rather overfilled tub into the machine. When I started the DreamStation, the whirring/whining began.  Annoyed-and-disappointed

This could not do. I wiggled the hose connection to make sure it was well seated. I checked the hose for a crack or a leak. I turned the machine off and back on (hoping that like for a computer, sometimes a simple reboot will cure what ails it). All to no avail.

Then I removed the humidifier tub, emptied it into the sink so that the water level was back down to the actual RTFM-indicated level, then used a few tissues to sop up the excess water in the inner tube-like structure in the tub, sopped up the water that had spilled into the base of the machine below the tub and around the heating plate, and for good measure sopped up the rectangular rubber seal edges all along the inner lid of the humidifier chamber.

It worked. No more noise. I don't know which one of the above measures did the trick or whether it was a combination of some or all of them, but I postulate that the key is don't overfill the humidifier tub, and make sure if you do that you sop up any spilled water and keep the seals water-free. That should keep the old girl quiet. Shhh

I have 16/12 for my settings. After reading many posts on this forum my pressures seem high compared to a lot of the posts I am reading? Does this indicate I have a lung problem or other health issue related to my breathing?

So with my non-wheezing A10, I'm noticing the central events are continuing. I've always had them, just never high enough to do anything about it. The CI is never higher than 5, usually less than 3. In the beginning, I had a LOT of pressure induced central events so we had to move to treatment pressure in stages. Since I was given a brick at that time, and had no way of knowing it was otherwise, I don't know how it all worked out until I got the S9 Autoset myself years later.

My settings are 15 - 20.
My 95% is between 17 and just under 19.
The max each night is usually between 18 and 20.
My AHI varies between 2 and just under 5.
CI is usually about half the total. Event time is from 10 to 17 seconds.
HI is barely there.

I take a LOT of medication, so one of them could be the cause. I had a migraine yesterday and took what I call the "Hammer" (rizatriptan / Maxalt) and went to bed. I slept about 5 hrs. The AHI for that session was 2.16 and all of them were central. I can list my meds here if anyone wants a gander.

So my questions are:
1 - While I am assuming this is kinda normal, and I am assuming since they are under 5 all is well, I am still concerned. Should I consider switching to a BPAP or even an ASV to address this?
2 - I am in the process of losing weight (heart insists on it) and my hope is my pressure needs will decrease. I am fairly certain the CI will not decrease with the weight loss but they might with the decrease in pressure. Should I wait it out? But we''re talking a year or more most likely (slow weight loss is best)
3 - am I worrying about nothing?

Places to get low capacity SD cards

I was surfing, looking for a FlashAir card. I came across some smaller capacity SD cards.

B&H Photo (online place) has 2gb and 4gb sizes. 2 is $7.99 and 4 goes up to $18
SecondWind (Supplier #2) has a 1gb for a ghastly $14.

Look for them at photo places, not computer places. They are getting harder to find and higher capacity ones are getting cheaper.

Here is a curious question that I was asked about a ResMed device.

Today is March 17, 2018 (Happy St. Patrick's Day) and 12:55 p.m. ...

(1)  Should the clock read March 18 because ResMed considers a new day to start at 12:00 p.m. or should it read the correct date March 17, 2018?

(2)  On March 17th, should MyAir show the correct sleep data displayed on the machine as a "Saturday, March 17) date or should it have been March 16th?

My original answer was that the clock should always be set to the current local date and time. -- However, the user can't set the clock back one day per the device's error detection process? So if the date is wrong, how does one overcome the machine's objections and reset the day backward by one?

Friends,

I am a 45 y.o. male diagnosed with moderate SA (AHI = 20) in Dec, 2017. I have been on a CPAP ever since.

I have noticed that I have no SA symptoms when taking a nap while lying on a slope (which is unsurprising). I would like to test the results of my sleep while lying on various slope angles and if, while at a reasonably low angle, my AHI drops significantly, I may start thinking of buying a raised/adjustable bed.

Now, I will only be able to do so if I have a polysomnograph handy.

Question: was anybody able to acquire a portable polysomnograph somewhere? The simplest possible (3-4 channels) will do.

NEED ADVICE FOR NEWBIE W NO MACHINE

Good Sleep Peoples, Please help!!!! I know I have central sleep apnea.. Unfortunately, had EEG and sleep study and both did not show apnea because I was awake for most of them but as I go through specialist after sleep specialist after cardiologist it has taken a very bad turn. I can't even nap without that electric shock of your body waking you and if it doesn't then I wake up with almost migraines and gasping for breath.

I had my husband watch me when I sleep and he said I have long instances of no breathing. I get maybe 3 - 4 hours of sleep a night. I can't work. I can't drive. I can't do anything. I feel like I am waiting to die. Sad((((((

Besides knowing WHY I have it (I am happy to chase that with the neurologists for the rest of the year) I ABSOLUTELY NEED TO SLEEP. I am fine once I get to REM but NREM is a disaster and opiates, benzos make these events WORSE. Sad(( 

The sleep doc said they would give me a prescription for a machine but 

1. The fitting place can't schedule me for months
2. Unsure if the prescript MUST be for ASV. 

I know this is what I need. How can I get an ASV (money is not an issue ATM bc, well, if I can't sleep or die in my sleep then none of htis matters)??? 

Can I buy one and THEN, after the second and third in-house week-long sleep study happens (can't FREAKING GET IN UNTIL JULY) what are the chances that insurance will reimburse?

Question about Insp. Time

Following advice I changed the pressure from 16/12 to 16/13. I changed start ramp pressure from 8 to 9 because I felt I was not getting enough air during the start of ramp. The change to start ramp pressure helped. The DME changed the mask from F20 large to F20 medium on Thursday. The leaks are high but getting better. 
My question has to do with Insp. Time. It is set to 2.0, I have noticed the pressure support dropping before I have finished breathing in. SleepyHead shows 95% insp. rate is 2.00.
Should I increase the Insp. Time, if so how much?

Can someone tell me if sleepyhead works with firstly a mac and then with the BMC Luna G11 machine please, I also want to know if I can read my CMS50D+ oximeter data with sleepyhead on the mac.

I have been on the CPAP since early this month.  I do not wake up more refreshed at all.  I had some low overall AHI nights (below 5) but overall not the improvement I was hoping for.  I seem to have many CA events (according to SleepyHead software).  Before I woke up this morning I was in CSR for 75 minutes.  The machine seem to do its job when it comes to reducing obstructive apneas.  When I did my sleep study I was diagnosed with OSA at a rate of 15.5 - so medium.  During my period with high volume of CAs, my projected AHI for that period is 38.  My snoring is pretty much gone, which makes the wife happy, but I got the CPAP machine for better sleep which so far seems to be eluding me.

What to make of this data - that is my question?

What can cause an O2 drop without an OA/CA event?

After switching to BIPAP for last 3 weeks, my AHI went way down. But I'm still having SpO2 desaturations during the night. Here is one desat that had no event flagged with it. Also at 5:36pm I had a 15sec UA, and that caused O2 drop to 84%, and HR went up to 97.

I know I need to get leaks under control, getting a new foam liner this week. I feel if i go to my sleep doctor about this, he would say "you're doing great", since AHI <1 most nights now. But those desats still wake me up.

Questions:
1. Why did O2 drop but no event flagged?
2. On the event at 5:36pm, is it normal for a 15sec event to cause a O2 desat to 84%?
